What This Website Collects

This website collects only what you type into the consultation request form on the Contact page: your name, your email address, your phone number if you share one, the service you are interested in, and your message. If you never fill in the form, the site does not collect any of that information.

Like most websites, the hosting provider may log basic technical data, such as page requests, for security and performance. That data is not used to identify you.

How It Is Used

Your request is used to reply to you and to arrange your free 15-minute consultation by phone. It reaches Jazmine's inbox through a form delivery service run by the company that manages this website, which keeps a backup copy so no request is lost and counts requests for basic reporting on how the site is doing. Your information is not sold or used for advertising.

Please Keep Sensitive Details for the Consultation

The request form is not a secure channel for clinical information. Please keep your message brief and leave out details about your health, history, or diagnoses. Those conversations belong in your consultation.

Links to Other Services

Some buttons open services run by others, such as Jazmine's intake form on Google Forms and her SimplePractice request page. Information you enter there is handled under those services' own terms, not by this website.

This Website and Your Clinical Records

This website is a way to learn about the practice and request a consultation. It is not a clinical record system. If you become a client, your records are kept separately by the practice and are governed by Jazmine's professional obligations as a Maryland Licensed Clinical Marriage and Family Therapist and by HIPAA.

Asking About or Deleting Your Information

You can ask what information this website collected about you, or ask for it to be deleted, at any time. Send a short note through the request form on the Contact page saying what you would like.
